New Patients

New patients are always welcome to our practice!

Please contact your primary care provider if your insurance requires a referral. To schedule or change an appointment with a physician or for a test, please call our office at (508) 771-4205. Please choose option 1 on our automated system and leave a message in the appointment mailbox. A staff member will call you back to book the appointment. If you need an urgent appointment please dial 0 for the operator and someone will assist you.


Medical Records

It is of the utmost importance that we have copies of your medical records available prior to your appointment. In order to assure this, you may be asked to help us obtain these records or sign a medical records release form which allows us to request them from your physicians.

The charge for your records is 25 cents per page

We encourage you to make your appointments as far in advance as possible. We recognize the importance of your time and will make every effort to meet your requests.

Confirmation

We will call you 2 days before your appointment to confirm and give you instructions as necessary. Please be sure we have your daytime phone number on file.

Cancellations and Missed Appointments

Cancellations

We understand that from time to time you may have to cancel or reschedule an appointment.

We ask that you make every effort to notify us as soon as possible or at least 24 hours in advance. If you are scheduled for a Nuclear Cardiology Test we require 48 hours notice for cancellations


In order to provide you with the highest standard of care, keeping patient appointments is crucial in this process. It is in your best interest to make sure you keep your appointments and show up at the scheduled time.

You may be charged a fee for each appointment or test that is missed or not cancelled with appropriate advanced notice.

Patients that miss two or more appointments in a calendar year without 24 hour notice, and without extenuating circumstances will be subject to dismissal from the practice with a 30 day notice of emergency coverage until established with another physician.

Insurance and Payment

Insurance

Heart Center accepts most major insurance plans and we will file your insurance claims for you. Please bring your insurance cards to your appointments. Please check with your insurance company before your visit to see if you require a referral or authorization from your primary care physician.


Patient’s Financial Responsibility

Patients are financially responsible for all charges not paid by insurance. Your copay is expected at the time of service. We accept Cash, Checks, VISA, and Mastercard.

COPAY


Copays are expected at time of office visit.

Cardiac testing may require copayments or deductibles and are expected to be paid in a prompt timely manner. Please check with your insurance company if you have any questions on the deductible and costs. This will avoid undue stress.

Returned checks

There will be a fee of $5.00 charged by this office for each check returned to us by your bank.

Collection Agency Costs

In the event that your account is forwarded to a collection agency, you agree to pay an additional fee equal to 25% of the balance forwarded to the collection agency and any additional attorney fees or court costs..


Patient assistance

If you have a financial hardship, please contact our Billing Department to arrange a payment plan at 508-771-8804.

Emergency and After Hours

Emergency

In case of an emergency, call 911 to dispatch an ambulance. Once you are admitted to the hospital, have the emergency room staff notify the on call physician.


After Hours

If, after hours, you need to reach the on–call physician for emergencies only, please call the main office number at (508) 771 4205 and ask the answering service to notify the doctor and have him call you.
